What Happened: NotPetya was a destructive malware attack that started in Ukraine in June 2017 but quickly spread globally, causing unprecedented damage. Initially masquerading as ransomware (demanding payment), NotPetya was actually a wiper – its payload irreversibly destroyed data on infected systems. It propagated via a Ukrainian tax software update (another supply chain vector) and through network exploits. Major global companies like shipping giant Maersk, pharmaceutical Merck, and courier FedEx/TNT were not even targeted specifically but became collateral damage, suffering massive outages. Maersk, for example, had to halt operations as NotPetya took down 49,000 of its PCs and servers across 600 sites in a matter of hours. The company’s ports and shipping operations were paralyzed, as they lost access to critical logistics systems.

In extreme cases, your team may be tasked not just with responding but with rebuilding – and that requires planning and prioritization (which applications to restore first, how to communicate with customers during outages, etc.). NotPetya also emphasized global cooperation; companies, governments, and cybersecurity firms shared information rapidly to contain the outbreak. Executives realized cybersecurity is not merely an IT issue but an enterprise risk that can halt all operations, which helped spur investments in resilience (like backup communications systems and more robust recovery testing).

Are there audits or reviews of incident logs to ensure proper procedure was followed? Governance involves oversight, and oversight is enabled by metrics and reports.

Another governance aspect is clarifying decision-making authority during incidents. The IR plan should spell out who has the authority to, say, shut down a production system to contain an incident (is it the CISO? the CIO? plant manager?). It should clarify when the incident commander needs to seek approval from higher-ups – for example, any decision to pay a ransom or any incident with large regulatory implications would typically involve the CEO and possibly the Board. Sorting this out ahead of time prevents confusion and delays. During Colonial Pipeline, for instance, the decision to pay ransom would have been elevated to the CEO and informed by federal authorities – a pre-existing policy can guide that elevation.

Lastly, governance includes training and awareness at the leadership level. Executives and directors should receive at least a basic orientation on the incident response plan and their roles in it. Tabletop exercises (simulated breach scenarios) that include executives can be extremely valuable; they not only test the plan but also educate leadership on the challenges and decisions they may face. Sometimes this also means paying for threat intelligence feeds or subscriptions to services like a malware sandbox for analysis. While it might be tempting to cut corners (security tools don’t directly produce revenue), savvy executives see them as insurance – much like one would invest in fire alarms and sprinkler systems in a building.
  • People: Incident response is labor-intensive. Organizations need skilled analysts, engineers, and possibly third-party consultants or retainers (for example, an agreement with a cybersecurity firm to provide surge support or incident response services if something big happens). For example, if the BC plan says that the ERP system can only be down for 4 hours before severe impact, then the IR plan for a cyber incident affecting the ERP should aim to isolate and recover that system within that window (perhaps by failing over to a DR environment or restoring from backups). This may influence technical decisions – maybe an isolated backup environment is maintained for ERP to use in emergencies.
  • Coordinate Teams and Communication: Often, organizations have separate teams for IT disaster recovery (handling power outages, etc.) and cyber incident response. In a major cyber crisis, they must work hand in hand. Executives should facilitate joint exercises between these teams. Aligning Incident Response with Frameworks and Standards (ISO, NIST, MITRE, COBIT)

Aligning with these frameworks ensures that the IR plan is comprehensive, meets compliance requirements, and follows a common language that stakeholders (including auditors, partners, regulators) can understand. Here’s how alignment with some key frameworks can be beneficial:

NIST Cybersecurity Framework (CSF) and NIST Special Publications:

The NIST CSF, widely adopted internationally, is built around five core functions: Identify, Protect, Detect, Respond, Recover. Incident Response planning lives mainly in the Respond and Recover functions, but it’s connected to the others. Aligning with NIST CSF means our incident response capabilities are part of a larger, balanced security approach. For example, under CSF’s Respond function, outcomes include executing response plans and communication (RS.CO), analysis of incidents (RS.AN), mitigation (RS.MI), and improvements (RS.IM). We should map our IR plan to these outcomes: do we have processes for analysis and mitigation? Do we incorporate lessons learned (improvements)? By doing a self-assessment against CSF categories and subcategories, leaders can identify gaps. Perhaps the assessment shows strong technical response but weak in external communications (RS.CO2 deals with reputation and public relations management in an incident). That insight can guide enhancements.

Moreover, NIST publishes specific guidance like NIST SP 800-61 (Computer Security Incident Handling Guide). Following 800-61’s recommendations (which include the four-phase IR life cycle we discussed ) practically guarantees a solid foundation. Revision 2 of 800-61 is older (2012), but Revision 3 is in draft as of recent times , indicating updated practices (like dealing with cloud incidents). Ensuring the IR plan is consistent with NIST guidelines also helps meet certain regulatory requirements that reference NIST. For instance, US federal agencies and contractors often must follow NIST standards, so a company in that supply chain aligning to NIST IR practices demonstrates due diligence.

ISO/IEC Standards:

ISO/IEC 27001 is the well-known standard for Information Security Management Systems (ISMS). It requires organizations to implement controls for incident management (Annex A.16 in ISO 27001:2013 deals with managing information security incidents and improvements). Aligning IR with ISO means formalizing the process: having incident registers, defined response procedures, and continual improvement cycles. There is also ISO/IEC 27035, a dedicated multi-part standard on Incident Management, which outlines how to establish an incident response process and team. ISO 27035 breaks incident management into preparation, identification, analysis, and so forth (similar to NIST, with a five-step model as we saw ). An organization seeking ISO 27001 certification will need to show auditors that they have an incident response procedure (e.g., evidence of incident reports, post-incident review reports, etc.). By aligning with ISO, you not only improve the effectiveness of IR but also gain credibility that your process meets an international benchmark.

For leadership, pursuing ISO certification or at least aligning with it can reassure customers and partners. Many RFPs or vendor risk assessments ask “Do you have an incident response plan aligned to a standard like NIST or ISO?” A “yes, and here’s how” can be a competitive advantage, strengthening brand authority as a trustworthy, well-governed organization.

MITRE ATT&CK:

While MITRE ATT&CK is not a process framework like NIST or ISO, it has become an invaluable framework for threat-informed defense. Aligning incident response with MITRE ATT&CK means utilizing it to map and understand attacks and to ensure detection/response coverage across the spectrum of techniques. For instance, a CISO might use ATT&CK to perform a gap analysis: list the top 10 techniques attackers are using (as per threat intel or the Picus report that showed 93% of attacks used top 10 techniques ) and then verify that the organization has the ability to detect or mitigate each of those. If “credentials from password stores (T1555)” is common, do we have a way to notice if an attacker dumped credentials? Maybe the EDR can alert on that. If not, that’s a gap to fix (perhaps by enabling additional logging or deploying specific detection rules). This is how ATT&CK alignment directly enhances IR – by preemptively positioning controls and detections for known attacker behaviors, the IR team is less likely to be blindsided.

MITRE ATT&CK is also useful for communicating with non-technical executives about what threats the organization is prepared for. A heat map might be presented showing which adversary techniques the company can handle well versus not. That can tie into risk appetite discussions. Leadership can set targets like “We want to be able to handle the top 90% of techniques used by threats to our industry” and resource accordingly. ATT&CK also aids in incident investigation: responders can label what tactics were observed in an incident, and then management can see if there is a pattern (for example, perhaps incidents repeatedly show successful phishing and privilege escalation – indicating a need for better MFA or user training).

COBIT (Control Objectives for Information and Related Technologies):

A company can assess its incident management maturity – is it ad-hoc, or is it optimized with continuous improvement? COBIT also emphasizes linking incidents to problem management (finding root causes) and ensuring knowledge gained is fed back into risk assessments.

For top management, COBIT provides a language to discuss incident response in terms of governance outcomes. One could ask: Do we have policies (Evaluate, Direct, Monitor) around incident response? Do we have the right management practices (Plan, Build, Run, Monitor) for our IR process? COBIT’s focus on aligning IT with business means that an incident response should always consider business priorities – which we’ve echoed in continuity alignment. It also encourages involving stakeholders in setting tolerance levels for incidents and defining what must be reported to whom – aligning with the earlier governance and communication points.

Regulatory and Other Frameworks: Depending on the industry, there may be sector-specific standards. For example, the financial sector might follow the FFIEC guidelines or PCI-DSS requirements (which mandate having an incident response plan for payment card data breaches). The health sector might follow HIPAA security rule which also requires incident procedures for ePHI breaches. Even frameworks like MITRE’s Shield (for active defense and deception) or the SANS Institute’s incident response cycle can be referenced. Ensuring the IR plan is in harmony with any such relevant frameworks keeps the organization compliant and up-to-date with best practices.

In practice, aligning with frameworks should not be seen as a burden but as a way to structure and verify your incident response readiness. It provides checklists and benchmarks. For instance, an audit against NIST or ISO might reveal that while the technical team is solid, the documentation is lacking (common in tech-driven organizations). Good documentation (policies, procedures, contact lists) is actually critical in crises, and frameworks force you to have them. On the flip side, frameworks can also prevent over-engineering by clarifying what’s essential.

Executives can champion framework adoption by stating a goal like “We aim to align our cybersecurity program with NIST CSF by end of next year” or “Let’s get ISO 27001 certified.” This rallying point can help galvanize the team and justify improvements, and ultimately, it results in a more systematic and defensible incident response capability.

Building a Culture of Preparedness: Training, Drills, and Continuous Improvement

The final piece of the puzzle is arguably the most important: culture. An organization can have all the policies, tools, and frameworks in place, but if the people are not engaged and prepared, the response will falter. Building a culture of preparedness means making incident response everyone’s responsibility at some level, and ingraining the mindset that cybersecurity is key to the organization’s mission.
